Follow-up policy after a consultation

1. PURPOSE

The purpose of this policy is to clarify the conditions surrounding free follow-up after a paid consultation for the same health problem, in order to ensure transparency and avoid confusion.

2. SCOPE

This policy applies to any patient who has had an initial paid consultation (either on-demand consultation or through a package) with a nurse practitioner (NP) at Clinique THVR inc.

3. DEFINITION OF FREE FOLLOW-UP

-Follow-up care at no extra cost consists of a single additional appointment to assess the progress of a health issue already addressed during the initial consultation.

-This follow-up is offered at no additional cost, only for the same health issue that was treated during the first appointment.

-Follow-up can be done by teleconsultation or secure communication, depending on NP’s clinical judgment.

 4. TERMS AND CONDITIONS

-Follow-up care at no cost must be provided within a reasonable time frame, determined by NP based on the nature of the health issue.

-If a new health issue arises during the follow-up, it will be the subject of a new consultation, for which a fee will be charged.

-Follow-up care at no cost does not include unlimited coverage: it consists of a single, one-time consultation directly related to the problem initially treated.

5. PATIENT RESPONSIBILITIES

-The patient must follow the instructions given during the initial consultation and schedule follow-up appointments according to the recommended schedule.

-If they miss a follow-up appointment, free care is no longer guaranteed and a new appointment may be charged.

-The patient understands that continued care beyond the free follow-up requires either a new on-demand consultation or a package (Individual, Family, or Premium).

6. DISCLAIMER

Follow-up care at no cost is intended to ensure safe and professional continuity. In the event of worsening symptoms or an emergency, the patient should immediately call 911 or go to the nearest emergency room.
